How to get a Mobility Citycard
If you are a permanent City resident with a qualifying disability you are eligible for a free Mobility Citycard. To get your card you need to fill out a Mobility Citycard application form. You can get one of these from the Broadmarsh Travel Centre in Broadmarsh bus station, any City library. (If you are not able to pick up an application form, contact the Travel Centre, on the number below, and they can send one to you) or the nct Travel Centre in Old Market Square.
Once you have a fully completed application form, take it to the Broadmarsh Travel Centre along with proof of your ID, City residence (accepted forms of proof are detailed in the application form) and proof that you are getting the Higher Rate Mobility Component of the Disability Living Allowance or War Pensioner's Mobility Supplement ( if applicable). A photograph will be taken of you for your pass and your Citycard will be ordered for you.

Mobility Citycard Plus
Applicants can apply for Mobility Citycard Plus if they are blind or their doctor has certified that they can only travel on public transport with the aid of a companion. The same application form is used.
Please note that most Mobility application forms for a companion pass will be referred for an independent medical assessment.
If you lose your Mobility Citycard
If you are unfortunate enough to lose your card, call the BroadmarshTravel Centre (on the number above) and they will check to make sure it hasn't been handed in. If there is no sign of it they will issue you with a new Citycard and your old card will be made unusable. There is a £5 charge for a replacement card.
